Double chin and jawline

A soft jawline can be fat, loose skin, jaw muscle, a short chin or enlarged salivary glands. Some need reducing, some need tightening, and one needs neither.

What usually causes it

    Fat under the chin

    Fat under the chin can be there at a normal body weight. Reducing it shows the jawline that was hiding.

    How we tell them apart

    • Pinching under the chin gives a thick, soft fold
    • It is there whether you look down or straight ahead
    • It did not change much with weight loss

    Loose skin

    Little fat, but the skin has lost its hold and hangs. Reducing fat here makes it worse. The skin needs tightening.

    How we tell them apart

    • The pinch is thin and loose, not thick
    • Worse when you look down, better when you lift your chin
    • Neck lines and crepey texture

    Jaw muscle and chin structure

    A wide masseter muscle squares the jaw. A short or recessed chin lets the neck skin fold. Neither is fat.

    How we tell them apart

    • The sides of the jaw bulge when you clench your teeth
    • You grind or clench at night
    • In profile, the chin sits behind the lips

    Enlarged salivary glands

    The glands below the jaw angle can be enlarged, giving a fullness that looks like fat but is not. Fat treatment does nothing here.

    How we tell them apart

    • Fullness sits just under the jaw angle, on both sides
    • Does not change with weight
    • Firmer than fat when pressed

Diagnosis comes first

What we look for first is the pinch test and the clench test. A thick pinch is fat; a thin, loose pinch is skin. If the sides of the jaw bulge when you clench, it is muscle. A soft fullness just below the jaw angle that does not change with weight can be the salivary gland.

Good to know

  • Fat plus loose skin is common. Reduce fat first, then tighten, or you are tightening over bulk.
  • Fat-dissolving injections swell for a while before they shrink. Do not book it the day before photos or a flight.
  • Muscle Botox works gradually, and the jaw can feel tired when chewing at first.

  • Is it fat or skin?

    Pinch it. Thick and soft is fat; thin and loose is skin. Many people are both, and the scan and exam settle it.

  • Fat-dissolving or Ultherapy?

    V-Olet reduces fat. Ultherapy tightens the skin and the layer under it. Fat-only cases lean V-Olet, loose-skin cases lean Ultherapy, and mixed cases get both in sequence.

  • Can I do this before flying home?

    Ultherapy and Shurink: minimal downtime. V-Olet: expect swelling for a while. Botox: none. We order the steps around your dates.
